NOTE: The requests with an * can only by made as a SINGLE request. For requests 3 through 6 you can choose multiple options, but these cannot be combined with requests that have an *. Please only use the "Other" option if none of the previous options is applicable. Because the workflow is unsure for this option it will cycle through all workflow steps (approval, registrar, advising, FLAC Liaison, and FLAC Team-HR).
Request Type
For Ticket Title please use the CRN, course SUBJ CODE and NUMBER, and the request type CODE you are making. The title should be organized as follows (note that request type codes can be found in the field above):
[CRN]: SUBJECT CODE & NUMBER (RT = #)

An example of a title using an existing course:
[1111]: SOC 250 (RT = 1)

An example of a title using a course being added:
[NA]: SOC 348 (RT = 7)
EX: SOC 250 - Introduction to Sociology
Please enter only the CRN of the course, nothing else (e.g., 1457). If this course does not yet have a CRN please enter "NA" for the CRN.
Courses normally vary between 1 and 4 semester hours.
Describe the workload or compensation change that needs to occur. Usually these types of changes are handled by the FLAC Liaisons and HR FLAC Team.
If this is an independent study course please attach a course outline or syllabus to this form using the Document Upload box below.
For courses that are 1) being cancelled, 2) converted to directed/indep. study, or 3) having a change in modality, please copy and paste the student roster in the box below, including student ID and email information. A student roster that includes email addresses can be found on banner using the SWASLST (Class Roster Email Query Form). This will help the advisors contact students to ensure they are notified of changes.

Type NA if the course has no students.
